How to Find In-Network Providers
This information is provided in an effort to assist participants with locating in-network providers. It is the responsibility of the participant to confirm with individual providers that services are in-network. Information on this site should be considered a starting point.
If you have a previous doctor, contact your provider’s office to ask if they accept Anthem Blue Access PPO or National PPO (BlueCard PPO).
Blue Access (PPO) is your plan if you are in one of the following 14 Anthem states: CA, CO, CT, GA, IN, KY, ME, MO, NH, NV, NV, NY, OH, VA or WI.
National PPO (BlueCard PPO) is your plan if you are currently in a Non-Anthem state, or any state that is NOT listed above.
Go online to anthem.com.
Click on the Individual & Family tab. A menu will drop down. Click on Find a Doctor. Once you select Find a Doctor, a new page will open. Choose Search as a Guest.
Then answer the following questions:
How do you get insurance?
Through my employer
What state do you want to search in?
What time of care?
Select a Plan/Network
Choose Blue Access PPO or National PPO based on your state listed in Option 1.
To Find a Pharmacy

Express Scripts members call 800.467.2006.
To Find a Dentist
If you have a previously established relationship with a dental office, contact your provider’s office and ask if they accept Delta Dental of Ohio.
To find a provider, visit deltadentaloh.com/hhwf. Select FIND A DENTIST. On the next page, select Delta Dental PPO and Delta Dental Premier. Then, select YES to search by location and click to find a dental provider.
To Find a Vision Care Provider
If you have an established relationship with a vision provider, contact the office to ask if they accept Vision Service Plan (VSP).
To find a provider, visit vsp.com. In the FIND A VSP DOCTOR box, enter your zip code and click search.
