If you have medical coverage with Heartland Health & Wellness Fund, to find out if your provider is a member of the Anthem network, complete one of the following options listed below:

Option 1

Contact your provider’s office to ask if they accept *Anthem Blue Access PPO -or- **National PPO (BlueCard PPO). *Blue Access (PPO) is your plan if you are in one of the 14 Anthem States (CO, CT, GA, IN, KY, ME, MO, NH, NV, OH, VA, WI, PR, VI).

**National PPO (BlueCard PPO) is your plan if you are currently in the NON-Anthem states (see above for Anthem States).

Option 2

Go online to anthem.com.

Select the “Find a Doctor” button (on the top of the page to the right).

Once you select the “Find a Doctor” button, a new page will open. You are given the options to “Search as a Guest” and then follow the prompts.

If you have a Heartland/Anthem medical ID card, you may “Search as a Member.” Choose “Search as a Member” and in the block labeled “Identification number or alpha prefix (first three letters):” type in the first three letters of your member ID (either UWE or UFI) and click “Continue.” Follow the prompts to locate a medical professional.
