Heartland Health & Wellness Fund partners with Anthem, Delta Dental and Vision Service Plan (VSP) to offer participants access to quality benefit providers. Eligibility for benefits is determined by the Collective Bargaining Agreement between your employer and your union.

Your medical benefits are provided by Anthem Blue Cross and Blue Shield. If eligible and enrolled for medical benefits, you should receive an ID card in the mail, or you can access a digital ID card through the Sydney Health App.

Active Kroger Participants: Your prescription drug benefits are provided by Kroger Prescription Plans. Contact them at 800-575-7712 or visit the Kroger Prescription Plan website.
All CVS Participants: Your prescription drug benefits are provided by CVS Caremark. Contact them at 866-284-9226 or visit the website.
All Other Participants: Your prescription drug benefits are provided by Express Scripts. Contact them at 800-467-2006 or visit the website.
Vision benefits are provided by Vision Service Plan (VSP). VSP will not send you an ID card, but you can access your information through their website. Contact them at 800-877-7195 or visit the website.
Dental benefits are provided by Delta Dental of Ohio. Delta will not send you an ID card, but you can access your benefits ID and network information through their web portal. Contact them at 800-524-0149 or visit the website.

Carrum Health
For Kroger participants only. Carrum Health is a negotiated healthcare network provided by Heartland, available to Kroger participants, dependents (18+), and pre-65 retirees currently enrolled in a medical plan that covers procedures that include knee, hip, spine, heart and bariatric surgeries at a discounted rate.
